TRAINING REQUESTS
· Retroactive - Susan Moyer, Monica Williams (PCHS) – MISA Groups and Group Skills, May 14, 2010, Smethport, Pa. Request for meals, mileage and registration fee ($60.00) expenses.
Purpose: Required 20 hours of yearly training for direct care certification
· Colleen Wilbur (PCHS) – PACDAA, June 2-4, 2010, Harrisburg, Pa. Request for meals, mileage, registration fee ($175.00) and lodging ($286.38) expenses.
Purpose: Required for PACDAA membership, legislative contacts, BDAP updates, OMHSAS updates
· Angela Milford (Jail) – Prison Risk Management, May 27, 2010, Hershey, Pa. Request for meals and mileage expenses. There is no registration fee.
Purpose: Updates for jails
· Melissa Gee (PCHS) – CCR Work Group/MH Planning Steering Committee Meeting, June 17-18, 2010, Harrisburg, Pa. Request for meals, mileage and lodging ( $199.78) expenses.
Purpose: Mandatory, required state MH reporting that will be attached to future allocations. Steering Committee allows Potter County to have input into the next
5-year planning process.
· Melissa Gee (PCHS) – IM4Q Annual Retreat, July 20 – 21, 2010, Harrisburg, Pa. Request for meals, mileage, lodging ($261.96) and registration fee ($65.00) expenses.
Purpose: Annual IM4Q updates. Will accompany consumer/monitor for her updates as well
· Donna Foust, Darcy Shutt, Sue Weber (PCHS) – Annual Apprise Regional Update Training, June 21, 2010, Clarion, Pa. Request for lunch and mileage only.
Purpose: Mandatory Apprise Regional update to discuss Medicare, Medicaid, PACE/PACENET and other insurance.

· Notification of resignation of Cheryl Powell as Part-time Secretary Clerk II in the Divorce Division of the Prothonotary’s office.
· Request by Amy Moshier, Prothonotary to temporarily extend on an emergency basis the workweek of Records Preservation Clerk, Nancy Chitester to 34 hours.
